    i am very rusty with my care plans , i used to be very good at them. i’ve .not worked on a ward for several years and now find my selve working in an establishment for clients with very complex health needs, the book you refer to sounds like it is just the thing i could use. i’m from the UK ,would the book be compatable for use in the UK many thanks.

    I have a question , when you are evaluating your care plan based on the clinical experience, do your evaluations have to be what actually happened? Or can you just predict evaluations for the goals for example.

    Mr Ali 43 years old admitted to the medical word where you are training, and your instructor asked from you to assess and follow up him.
    Mr Ali is married has 4 children and work in his own grocery
    Upon assessment Mr Ali were complaining from the following health problems
    Vomiting , abdominal pain and frequent diarrhoea. Started about 10 hours before coming to hospital
    He has temperature 38.8℃ ,sweating B/P 145/95mmhg , he said that he is very tired and feels dizzy , and reported that he has allergy to acamol.
    He said that he is hypertensive and on Norvasc 10 mg daily, his Skin and mouth are dry ,seems not bathed and has smell
    Iv G/S 500ml iv to be started
    Lab tests CBC, Electrolytes and ECG to be done
    Please explain fo me .. how can I write 2 or more actual nursig diagnosis threw part statement???
